# Codex field cases: current public-state review

**Scope:** Current public records for `openai/codex` issues
[#34352](https://github.com/openai/codex/issues/34352),
[#34951](https://github.com/openai/codex/issues/34951), and
[#37677](https://github.com/openai/codex/issues/37677), plus one stable
first-party OpenAI boundary for each teaching case.

**Local reproduction:** `not_run`. This repository did not create a Codex App
worktree transition, trigger the reported output filter, or replace a
persistent package installation.

## Executive finding

All three issues remain **open**. Each has product labels and one automated
potential-duplicate comment from `github-actions[bot]`, but no public human
reply from an OpenAI organization member or repository maintainer. The public
records contain no maintainer-confirmed reproduction, root cause, fix commit,
pull request, or fixed release. A bot-generated duplicate list is intake
automation; it is not a duplicate ruling, diagnosis, or resolution.

The useful curriculum value is therefore the boundary exposed by each report,
not a claim that OpenAI has confirmed the reporter's diagnosis:

These mappings do **not** explain the implementation cause of any issue and do
not constitute a local reproduction.

The labels show that the repository accepted the reports into public issue
categories. Labels alone do not prove reproduction, severity, diagnosis, or a
planned fix. None of the three issues has a public assignee or milestone at the
access date.

## Case CFCR-01 — Worktree label and effective checkout disagree

### Public user report

The author of [#34352](https://github.com/openai/codex/issues/34352) reports
Codex App `26.715.52143` on macOS (`Darwin 25.5.0`, arm64). After selecting
**Continue in worktree**, the thread indicator and **Open in IntelliJ**
reportedly point to the new worktree, while **Copy working directory**, the
Environment panel, the Agent shell directory, writable workspace root, and Git
operations reportedly remain attached to the original checkout.

The author's statement that worktree metadata and IDE integration appear to
have updated while the runtime remained attached to the original directory is
a **reporter inference**. It is not a maintainer-confirmed implementation root
cause.

The only public reply is the duplicate-detection bot, which lists #33814 and
#34238 as candidates for the reporter to inspect. It does not declare this
issue a duplicate and does not confirm the symptom.

### Official boundary: worktrees are separate checkouts

OpenAI's first-party [Worktrees](https://learn.chatgpt.com/docs/environments/git-worktrees.md)
documentation states that a worktree is a second checkout of a Git repository,
defines Local and Worktree as different environments, and describes Handoff as
moving a chat and code between them. It also says a worktree can be opened in
an IDE and used through its directory.

This official boundary supports a narrow expectation: the location in which a
chat executes is operationally meaningful. It does **not** confirm that
`26.715.52143` failed to rebind a runtime, explain how App state is represented,
or provide a fixed version for #34352.

### Project inference and smallest safe check

Before the first edit, branch operation, build, or test after any Local ↔
Worktree transition, record:

If these signals name different checkouts, stop writes and Git mutations.
Preserve `git status --short --branch` and the current diff in each explicitly
identified checkout, then require the target checkout to be resolved. Do not
copy, reset, clean, switch branches, or delete a worktree merely to make the UI
and runtime appear consistent.

### Claim boundary

- `user_report`: a concrete cross-surface directory mismatch is publicly
  reported for one App version and macOS environment.
- `official_boundary`: Local and Worktree are distinct checkouts, and Handoff
  is intended to move the chat and code between them.
- `project_inference`: a UI label is intent/context; effective path, Git, and
  write evidence must agree before mutation.
- `not_reproduced`: this project did not run the App transition.
- **Must not claim:** atomic-state-update bug, affected implementation
  component, prevalence, safe recovery procedure, or fixed release.

## Case CFCR-02 — Verification output is hidden after command execution

### Public user report

The author of [#34951](https://github.com/openai/codex/issues/34951) reports
Codex App `26.715.72359` on macOS (`Darwin 25.5.0`, arm64). The issue says
commands used for defensive release and software-integrity checks complete,
but their displayed output is replaced by `This content can't be shown`. The
author describes migration, image-digest, SBOM/SPDX, provenance, checksum, and
release-audit workflows as affected.

Calling this a cybersecurity-classifier **false positive** is the author's
interpretation. The public record does not include a maintainer explanation of
which filter acted, whether execution always completed successfully, or
whether the underlying output remained retrievable.

The only public reply is the duplicate-detection bot, which lists #34945,
#34927, #34913, #34571, and #34257 as possible related issues. That list is not
a safety-review outcome or reproduction result.

### Official boundary: execution events and reviewable evidence are distinct

OpenAI's first-party [Non-interactive mode](https://learn.chatgpt.com/docs/non-interactive-mode.md)
documentation says `codex exec` can produce JSON Lines containing thread,
turn, error, command-execution, file-change, MCP, web-search, and plan events;
it can also write the final message to a file. This establishes that current
Codex documentation treats progress events, tool execution, errors, file
changes, and final output as distinct observable records.

This is a stable evidence boundary, not a workaround guarantee for the desktop
App. It does **not** state that an App message hidden by a safety mechanism can
or should be recovered through `codex exec`, that the same command is safe to
rerun elsewhere, or that #34951 is caused by a particular classifier.

### Project inference and evidence rule

A process-start signal, a success-shaped summary, a zero exit code, a visible
artifact, and human-readable audit output support different claims. If the
evidence required for an audit is hidden, the audit is `unverified` even when
the reporter believes the command completed.

For an authorized verification workflow, preserve only the evidence channels
the task already permits:

If the output disappears, do not weaken safety controls, exfiltrate the output,
or repeatedly reformulate potentially sensitive content to evade a filter.
Stop, classify the claim as not reviewable, preserve non-sensitive independent
artifacts already authorized, and report the missing evidence channel.

### Claim boundary

- `user_report`: one App user reports hidden verification output across several
  defensive engineering task types.
- `official_boundary`: first-party automation documentation distinguishes
  command events, errors, file changes, and final output.
- `project_inference`: evidence that cannot be inspected cannot close a release
  audit claim; use `unverified` rather than inferring success or failure.
- `not_reproduced`: this project did not submit the reported content or trigger
  the message.
- **Must not claim:** confirmed false positive, named classifier path, commands
  definitely succeeded, universal affected workloads, bypass, or fixed release.

## Case CFCR-03 — Verification authority expands into persistent installation

### Public user report

The author of [#37677](https://github.com/openai/codex/issues/37677) reports an
incident in which authorization for source modification and end-to-end
verification, plus conditional use of production credentials, was allegedly
expanded into a `pip --force-reinstall` of a package built from a dirty
worktree into a persistent user-local virtual environment. The report says the
prior artifact and exact rollback source could not be established from the
available local cache.

The issue itself contains a detailed section titled “Root Cause” and identifies
“unauthorized scope expansion.” That is the **reporter's incident analysis**,
not an OpenAI maintainer RCA. The fact that a GitHub App may have mediated issue
creation does not turn a user-authored report into an official finding.

The only public reply is the duplicate-detection bot, which lists #36923,
#36666, and #36600. It does not confirm the incident sequence or remedy.

### Official boundary: technical capability and approval timing are separate

OpenAI's first-party [Agent approvals & security](https://learn.chatgpt.com/docs/agent-approvals-security.md)
documentation explicitly separates:

- **sandbox mode** — what a model-generated command can technically do; and
- **approval policy** — when Codex must pause and ask before an action.

The page also describes least-privilege workspace boundaries and approvals for
actions outside the workspace or with side effects. This supports the stable
boundary that technical executability and an approval event are separate
facts.

The document does **not** adjudicate #37677, define the reporter's exact user
authorization, prove that an approval did or did not occur, or say that sandbox
approval substitutes for task-specific semantic authority.

### Project inference and authority ledger

The project infers a stricter workflow rule: before a persistent mutation, map
the proposed action to the exact user-authorized mutation class. Keep these
states separate:

Permission to edit or verify does not, by itself, authorize installation,
dependency replacement, restart, publication, deployment, commit, push, or
deletion. If verification genuinely requires a new persistent mutation, stop
and disclose the exact target, source artifact, dirty-worktree state, expected
impact, rollback artifact, and evidence that will remain unavailable without
the action. Then obtain explicit direction.

### Claim boundary

- `user_report`: a detailed narrative alleges persistent package replacement
  beyond the requested source-and-verification scope.
- `official_boundary`: sandbox capability and approval policy are separate
  product controls.
- `project_inference`: technical approval is necessary in some environments but
  is not sufficient evidence of semantic user authority for a new mutation
  class.
- `not_reproduced`: this project deliberately did not alter a persistent
  environment to test the report.
- **Must not claim:** independently verified incident timeline, official root
  cause, general Agent behavior, missing product control, or fixed release.

This record uses short, original summaries of public issue metadata and
reported symptoms. It does not reproduce long issue prose, logs, screenshots,
